We reply on the next business day with times that fit, and you arrive at a 60 to 90 minute candidacy evaluation that ends with a clear answer and a written figure. If Ortho-K is not right for your eyes, you will hear that at the same visit rather than after you have paid for lenses.
What the Form Asks For
What information does the consultation request form collect?
Five fields: your name, your email address, your phone number, the city you are travelling from, and the times that suit you. That is the entire form. It exists to start a conversation and reserve a slot, not to gather a medical history.
We ask for the city because drive time genuinely matters. The first month of an Ortho-K program includes several visits, and knowing you are coming from Gilbert rather than Ahwatukee changes how we cluster your appointments. We ask for preferred times so that our first reply can offer something workable instead of starting a round of scheduling messages.
What Happens After You Submit

The page confirms the request reached us. If you do not see that confirmation, the request did not send, so please call rather than assuming.

A member of the team contacts you by phone or email, using whichever you listed, and offers appointment times that fit what you told us. Requests submitted over a weekend are answered on Monday.

We confirm the appointment and what to bring
Once a time is agreed you receive a confirmation with the address, suite number, parking notes, and the short list of things to bring.

You attend the candidacy evaluation
Sixty to ninety minutes. Refraction, corneal topography of both eyes, tear film assessment, and a corneal health check, plus baseline axial length for children.

You leave with an answer and a written figure
Whether Ortho-K suits your eyes, what result is realistic, and what your program would cost. No pressure to decide in the room.
What to Bring
- Your current glasses, and your contact lenses if you wear them. Bring the boxes or the prescription if you have them.
- A list of medications, including eye drops and anything taken for allergies.
- Your insurance card, if you have vision or medical coverage you want us to check. Most plans treat Ortho-K as elective. See Insurance.
- Recent eye records, if you have had an exam elsewhere in the past year and can bring the report.
- For a child, the parent or guardian. Consent and the handling conversation both need you in the room.
- Sunglasses. Some tests are more comfortable afterward if you have them, and Phoenix afternoons are bright.
How Long the Visit Takes and What It Costs
Plan for 60 to 90 minutes. Most of that is measurement rather than waiting: refraction, topographic mapping of each eye, tear film assessment, corneal health evaluation, and for children a baseline axial length reading. The remainder is the conversation, which is the part most patients say they valued most.
A candidacy evaluation starts at $250 and is credited toward your program if you go ahead. A complete Ortho-K program starts at $1,800, and an ongoing annual myopia control program for children starts at $1,200. Your exact figure depends on your prescription and corneal complexity, and you receive it in writing before you commit to anything.

Frequently Asked Questions
Why does the form ask for so little?
Because a web form is the wrong place for clinical information. Collecting only what is needed to contact you and reserve a time keeps your health information out of an unsecured channel.
Everything clinical is gathered at the appointment, where we can ask follow-up questions and actually examine your eyes.
Is the consultation free?
No. It starts at $250 and it is a real diagnostic appointment with corneal topography, not a sales meeting.
The fee is credited toward your program if you proceed, so for patients who go ahead it is not an additional cost.
Should I bring my child to their own consultation?
Yes, and a parent or guardian needs to attend as well. We assess the child, but the conversation about handling, routine, and supervision involves you.
It also helps to know whether the child actually wants this. That is a genuine predictor of how the first month goes.
How do I cancel or reschedule?
Call (480) 706-3937 as far ahead as you can. Consultation slots are long, so an early cancellation lets us offer the time to someone else.
Please do not cancel by replying to a form confirmation, since that inbox is not monitored continuously.
Will I have to decide at the appointment?
No. You leave with a written figure and a clear explanation, then decide in your own time.
